Customer request

• Complaint: Disagreement related to products and/or services.
• Complaint: Disagreement not related to products and/or services; or, discomfort or dissatisfaction with customer service.

• The formulation of a complaint does not exclude the use of other methods of dispute resolution nor is it a prerequisite for filing a complaint with Indecopi.
• The provider must respond to the claim within a period of no more than fifteen (15) calendar days, with the possibility of extending the period for an additional fifteen days.
• By signing this document, the client authorizes to be contacted after the processing of the claim to evaluate the quality and satisfaction of the claim handling process.
